About Pharma²⁸

Connecting biotech innovation with rare disease opportunities worldwide.

Pharma²⁸ is a rare disease intelligence and market access platform based in Kendall Square, Cambridge, at the center of the Boston biotech ecosystem. We connect U.S. biotech innovation with clinical, regulatory, data and patient-access opportunities in Brazil and other international markets, supporting the responsible development of therapies for rare and ultra-rare diseases.

Why Pharma²⁸ exists

Closing the gap between scientific promise and patient access.

Promising therapies often face delays not because the science lacks potential, but because the path from innovation to patient is fragmented.

Biotech companies may need the right investigators, patient cohorts, clinical centers, regulatory pathways, data strategy, local partnerships and market-access intelligence before a therapy can move meaningfully toward patients in international markets.

At the same time, patients and physicians in large healthcare systems may face limited access to innovative therapies, especially in rare and ultra-rare diseases.

Pharma²⁸ exists to help close that gap. We provide the bridge between biotech innovation and the clinical, institutional, regulatory, data and patient-access ecosystems that can help rare disease therapies move forward responsibly.

Why Brazil

Our first strategic market.

With a single-market population of more than 200 million people, Brazil offers a powerful combination of scale, clinical diversity, experienced investigators, advanced academic hospitals, large public-health programs and significant unmet need in rare and ultra-rare diseases.

For biotech companies, Brazil can be more than a commercial destination. It can be a clinical development advantage — a place to identify patients, engage expert physicians, generate real-world evidence, explore regulatory pathways and build long-term access strategies.

But Brazil's strengths are often fragmented across institutions, specialties, public and private systems, academic centers and regional networks. Pharma²⁸ helps organize that complexity into a more structured, credible and actionable pathway.

Our role

A strategic bridge, not simply a market-entry advisor.

Pharma²⁸ is not simply a market-entry advisor. We operate as a strategic bridge connecting biotech companies with the physicians, institutions, patient populations, regulatory pathways, data assets and local expertise needed to evaluate and activate rare disease opportunities.

Our work is designed to make international markets more clinically legible, operationally feasible and strategically useful for companies developing therapies in areas of high unmet need.

Built in Kendall Square. Connected to Brazil. Designed for global scale.

Pharma²⁸ is based in Kendall Square, Cambridge, at the center of the Boston biotech ecosystem.

From this position, we connect U.S. biotech companies with Brazil's clinical and academic ecosystem, while supporting rare disease innovation across additional international markets over time.

Our first focus is Brazil. Our broader mission is global: to help promising therapies reach the patients, physicians and healthcare systems that need them.

Leadership and medical network

Leadership

Pharma²⁸ was founded by Fabio Sgarbi, Founder & Managing Director, to connect biotech innovation with international rare disease opportunities through a model that combines market strategy, clinical access, data intelligence and institutional partnerships.

Carlos Poyares Jardim, MD, PhD, serves as Medical & Scientific Advisor to Pharma²⁸, supporting the company's clinical and scientific perspective in rare diseases, pulmonary medicine, academic medicine, physician engagement and Brazil's healthcare ecosystem.

Pharma²⁸ also works with experienced physicians, physician-scientists, academic leaders and strategic healthcare partners to support rare disease programs with the clinical credibility and local insight required for responsible international development.

Our principles

Five principles guide Pharma²⁸.

We believe rare disease development requires more than access. It requires clinical depth, responsible strategy, trusted relationships, evidence generation, patient identification and a clear understanding of local healthcare realities.

  1. 01

Put unmet medical need at the center

    The patient defines the priority.

  2. 02

    Build with clinical credibility

Trust is earned through medical and scientific rigor.

  3. 03

Use data to make complex markets more actionable

    AI-enabled infrastructure supports human judgment.

  4. 04

Align biotech innovation with responsible patient access

Innovation must translate into real-world benefit.

  5. 05

Create international pathways that can compound over time

    Each program strengthens the next.
